ARC offers physician groups, hospitals, long-term care facilities, home health agencies and others in the community the opportunity to understand existing evidence-based models for reducing re-admissions and the fidelity required to successfully implement these models along with the very tactical and practical ways to implement them. Avoid Readmissions through Collaboration (ARC) is a 2 year project to improve care coordination and reduce re-hospitalization among various health care providers. Funded by the Gordon and Betty Moore Foundation, with supplemental support from the California HealthCare Foundation, ARC links hospitals with partners in long-term and ambulatory care to avoid preventable readmissions.
