Avoid Readmissions through Collaboration (ARC)

A two-year project that offers physician groups, hospitals, long-term care facilities, home health agencies and others in the community the opportunity to understand existing evidence-based models for reducing readmissions and the fidelity required to successfully implement these models along with the very tactical and practical ways to implement them.
