Meteor Program

Overview

The Meteor Program is designed to assist physician groups and independent practice associations to measurably improve performance on key Medicare Stars measures in 2011. It is designed for groups with existing registries, early in their Stars improvement planning, that want a structured and fast-paced environment in which to strengthen their patient and physician outreach to close care gaps.

The first in-person session will be scheduled for the week of July 25 in the LA area. Pre-work and 1:1 calls will take place prior to the first session to prepare groups.

Benefits

  1. Receive assistance in prioritizing Star measures and selecting interventions/changes to achieve improved performance in 2011
  2. Access to resources, tools and practical strategies from CQC and other CA physician groups working in this area
  3. Receive one-on-one coaching and technical assistance from CQC and other consultants throughout the project
  4. Facilitation of relationships with health plans to support data exchange and identification of common priority Star measures
  5. Support developing a measurement plan to evaluate your organization's progress
  6. Develop the necessary staffing models to support the selected interventions/changes
