Join the Program

Participation Requirements

  • Complete and submit the program application
  • Assemble a team of a minimum of 2 including a clinical leader and project manager (an IT designee is also highly recommended)
  • Provide evidence of CEO support (letter is included in application packet)
  • Submit program fee (see below)
  • Attend four onsite learning sessions, and in between, teleconferences and coaching calls
  • Share openly with other participating groups what changes/improvements you are testing and what you are learning
  • Submit quarterly data on performance in selected measures - a data reporting template will be provided

Cost

Program fees will be based on organization membership, and covers attendance of up to 5 individuals at each on-site session. For MSOs that are interested in registering multiple groups for the program, please contact Giovanna Giuliani to discuss pricing at ggiuliani@pbgh.org or (415) 615-6377.

The fees for the program are as follows:

  • Under 30,000 enrollees: $5,000*
  • 30,000+ enrollees: $10,000*

* You may select one of the following options for payment:

  • 50% of the program fee due April 2012, and the remaining 50% due January 2013
  • 100% of the program fee due April 2012
